Diabetes Self Management Education

We will not keep you in the dark. More than treating you, we believe in educating you. Diabetes self-management education and support at Family Tree Clinic is patient centered, respectful, and responsive to individual patient preferences, needs, and values and forms the basis for clinical decisions . Over a period of time, this will result in improved outcomes and reduce costs. We will help you understand the basics of Diabetes and Hypertension which is very crucial for managing Diabetes:

For example:

  1. How does the medication work?

  2. Should this medicine be taken with or without food?

  3. Does it matter what time of the day I take this medication?

  4. What side effects are most common?

  5. Can I drink alcohol with this medication? etc


Nutrition Therapy

For those clients with Diabetes and Hypertension, we strongly believe that the saying "food is equal to medicine" is rightly applicable. We engage a special nutritionist who will support you with healthful eating patterns. More than a Diet plan, we believe in Eating Right Plan. In the end, you have the answers for your basic queries like

  1. What are Carbohydrates, Proteins, Fats, Micro-nutrients and Fibre

  2. How to read the labels on foods

  3. How to calculate calories and follow portions

  4. Which sweeteners to use and which you should not


Physical Activity

Physical activity is another important part of our Lifestyle Management Plan. When you exercise, your muscles use sugar (glucose) for energy. Regular physical activity also helps your body use insulin more efficiently.

  3. Continuous Monitoring Plan.


Psycho-social Care

Timely assessment for symptoms of diabetes distress, depression, anxiety, disordered eating, and cognitive capacities using patient-appropriate standardized and validated tools at the initial visit, at periodic intervals, and when there is a change in disease, treatment, or life circumstance. We prefer Inclusion of you family members in this assessment.
